Make a Referral

This form allows anyone, family members, healthcare providers, community partners, or friends, to refer a senior in need of care to VCC PACE. Simply provide your contact information along with the details of the person you’re referring, including their name, phone number, and whether they have Medi-Cal, and let us know who we should follow up with.

Once submitted, our team will reach out to guide the referred individual through the next steps toward receiving comprehensive medical and social support that helps seniors live safely and independently in their own community.
