Vista Community Clinic PACE (VCC PACE) is the Program of All-Inclusive Care for the Elderly offered by Vista Community Clinic, a nonprofit community health organization founded in 1972. From its early days as a volunteer-run clinic, Vista Community Clinic has grown into a federally qualified health center dedicated to advancing community health for those who need it most. VCC PACE extends that mission to seniors across Los Angeles County.
Who We Serve
PACE stands for Program of All-Inclusive Care for the Elderly. It is a managed health plan built around the needs of older adults who would otherwise need the level of care a nursing facility provides, but who would rather remain in their own community.
Find out if VCC PACE is right for you
A little history of PACE
The PACE model began in the early 1970s with On Lok Senior Health Services in San Francisco, created so that families could care for elders without placing them in nursing facilities.
After years of success, Congress made PACE a permanent part of Medicare and a Medicaid state option through the Balanced Budget Act of 1997.
Today, PACE programs across the country help older adults age in their own communities, and VCC PACE brings that proven model to Los Angeles County.
Our approach to care
VCC PACE takes a person-centered approach that balances the medical, emotional, physical, and social sides of aging. We look at the whole person, not just a diagnosis, and we include you and your family in building and approving your plan of care. Because so many services are organized in one place, participants spend less time traveling from office to office and more time living their lives, and families feel some of the pressure lifted.
Your interdisciplinary team
At the heart of VCC PACE is an interdisciplinary team that manages each participant’s plan of care. When your care team shares information about your health and progress, they can offer coordinated treatment and recommendations.
Your team may include:
Primary care physician
Nurses
Social workers
Physical and
occupational therapists
Dietitians
Home care staff
Recreational therapists
Transportation
coordinators
Personal care
attendants
