Program of All-Inclusive Care for the Elderly (PACE)

Helping seniors in Los Angeles County stay independent, safe, and connected at home.

Vista Community Clinic PACE (VCC PACE) helps seniors keep living in the communities and neighborhoods they love. Our program provides all the medical care and support services you need to stay healthy, independent, and safe in their
community.

What is PACE, and who is it for?

PACE stands for Program of All-Inclusive Care for the Elderly. It is a managed health plan built around the needs of older adults who would otherwise need the level of care a nursing facility provides, but who would rather remain in their own community. You can read more on our About VCC PACE page.

The VCC PACE Difference

VCC PACE is a managed care health plan. We accept Medi-Cal, a combination of Medi-Cal and Medicare for those who are dually eligible, or private payment.

What you pay depends on your eligibility under Medicare and Medi-Cal. Our team will help you understand your specific coverage and any potential costs.

The VCC PACE Difference

Navigating health care can feel overwhelming, especially without someone in your corner. VCC PACE becomes that someone. Instead of juggling separate doctors, pharmacies, and specialists on your own, you have one
coordinated team that communicates, shares your care plan, and helps you make informed decisions. Vista Community Clinic has served Southern California
communities since 1972, and VCC PACE brings that same patient-centered approach to seniors across Los Angeles County.

No cost to ask

We’re here to help you explore your options.

Medi-Cal (No Share of Cost)

There is no cost to you if you have Medi-Cal with no share of cost. However, participants with a Medi-Cal share of cost remain responsible for that specific amount, and those without Medi-Cal are subject to a monthly premium. Ultimately, your individual cost is determined by your Medicare and Medi-Cal eligibility.

Other Eligibility

Participants with a Medi-Cal share of cost remain responsible for their individual share. Those without Medi-Cal pay a monthly premium covering long-term care and Medicare Part D drugs.

    Frequently asked questions

    What is PACE?

Vista Community Clinic (VCC) Program of All-Inclusive Care for the Elderly (PACE) is a managed healthcare plan exclusively for seniors. As part of VCC PACE, you receive primary medical care, medication management, physical therapy, specialty services, home care, social work assistance, and transportation to and from appointments. With VCC PACE, you can remain living in your own community and get the medical care you need in a safe environment.

VCC PACE is specifically designed for seniors 55 years and older who have chronic medical conditions and are struggling to live independently in the community.

You may qualify if you are 55 or older, live in our Los Angeles County service area, can live safely at home with our support, and meet the skilled nursing level of care requirements set by the California Department of Health Care Services.

No. PACE is designed to help you remain in your own home and community for as long as it is safe to do so.

If you have Medi-Cal with no share of cost, there is no cost to you. Those with a Medi-Cal share of cost remain responsible for that amount, and participants without Medi-Cal pay a monthly premium. What you pay depends on your Medicare and Medi-Cal eligibility.
