Home Care

Support that helps you stay safely in the home you love.

Staying in the community is the heart of the PACE promise. Unlike many health programs, Vista Community Clinic PACE (VCC PACE) provides community-based assistance that helps participants manage daily life safely and comfortably in their own surroundings.

It starts with an assessment

When you join VCC PACE, a home care nurse evaluates your home and your daily routine to understand where support would help most. The findings become part of your overall care plan, so your home and community support is coordinated with your medical care, therapy, and social services rather than handled separately.

How home care helps

Depending on your needs, VCC PACE home care may include:

Personal care, such as help with bathing and grooming
Light housekeeping to keep your living space safe and
comfortable
Help with grocery shopping and everyday errands
Support that reinforces the goals in your care plan

Care that adjusts as you do

Your needs may change over time, and your home care can change with them. Because your home care staff and your medical team share one plan, adjustments are made thoughtfully and in coordination with the rest of your care. The aim is always the same: to help you remain independent and secure in your own home and community.

Peace of mind for families

Home care also brings reassurance to family members and caregivers who want their loved one to be safe but cannot always be present. Knowing that a coordinated team is checking in and providing hands-on support can ease the worry that often comes with caring for an aging parent or spouse.
