Good medical care is the foundation of staying well in the community. At Vista Community Clinic PACE (VCC PACE), your care is delivered by a team of professionals who specialize in aging and chronic conditions, and who coordinate every part of your health so nothing falls through the gaps in care.
How it works
When you join VCC PACE, you are matched with an interdisciplinary care team. Your primary care provider, nurses, therapists, dietitians, social workers, and home care staff meet regularly to build a care plan that is right for you, and they update it as your needs change. Because the team shares one plan and one set of records, your care stays consistent and connected.
A full team of professionals
Alongside your primary care provider, VCC PACE participants have access to specialists who address the health concerns that matter most as we age, including:
Audiology, for hearing health
Dentistry, for teeth and gums
Optometry, for vision care
Podiatry, for foot care
Behavioral health, for well-being
When you need to see a specialist, your VCC PACE primary care provider coordinates. This keeps everyone informed and helps you receive the right care at the right time.
Care when and where you need it
VCC PACE offers virtual telehealth visits to help participants who are immunocompromised or who have trouble traveling. If a medical concern comes up outside of regular center hours, our nursing staff is available to help around the clock so you are never left without support.
Your built-in advocate
With VCC PACE, you do not have to navigate the health care system alone. Your team works together on your behalf, helping you understand your choices and making sure your care reflects your goals. All of your health care, including specialty services, is provided by VCC PACE or a provider authorized by VCC PACE.
In an emergency
If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Emergency services are handled differently from routine and specialty care. Please let VCC PACE know as soon as possible after any emergency so your care team can help coordinate your follow-up care.
